Telehealth Informed Consent

This document describes telehealth care provided by the independent provider groups (Online Medical Care, P.C.; TMD of Kansas, P.A.; TMD of Texas, P.A.; and TMD of CA, PC). You will be asked to consent to telehealth before a provider reviews your information.
What telehealth is
Telehealth uses secure technology to let a licensed provider evaluate and care for you remotely. Depending on your situation and applicable law, your visit may be asynchronous (you submit information the provider reviews) or synchronous (a live audio or video visit). The format is determined by the provider group in its independent medical judgment.
Benefits and limitations
- Telehealth can make care more convenient and accessible.
- Telehealth has limits. A provider may determine that an in-person visit, lab work, or other care is necessary, and may decline to prescribe.
- A consultation does not guarantee a prescription or any particular outcome.
- As with any care, there are risks, including that information shared remotely may be limited and that technical issues can occur.
Your choices
- You may seek in-person care at any time.
- You may opt out of using a given provider group or pharmacy in your account.
- You may withdraw this consent at any time, which may end your ability to receive care through the service.
Medications
If a provider prescribes a medication, it is dispensed by an independent, licensed compounding pharmacy. Some medications are compounded and are not reviewed or approved by the FDA for safety, effectiveness, or quality.
Privacy
Your health information is protected as described in the Notice of Privacy Practices. Information is handled within the United States.
Emergencies
Telehealth is not for medical emergencies. If you are experiencing an emergency, call 911.

Who does what
Velri is a brand operated by Velri Management LLC, a Nevada limited liability company. Velri is a non-clinical technology and administrative services company. Velri does not provide medical advice, diagnosis, treatment, or pharmacy services, and does not direct the clinical judgment of any provider.
All medical consultations and prescriptions are provided directly by independent provider groups (Online Medical Care, P.C.; TMD of Kansas, P.A.; TMD of Texas, P.A.; and TMD of CA, PC), by a provider licensed in your state. Velri does not employ providers. Velri's platform runs on technology provided by Telegra MD LLC, a non-clinical technology company that connects you with the provider groups and pharmacies. A prescription is issued only if a provider determines it is clinically appropriate in their independent medical judgment.
All prescription products are dispensed by independent, licensed compounding pharmacies. Velri does not own, operate, or control any pharmacy and does not dispense medication. You may opt out of using a given provider group or pharmacy at any time.
